From the Clinic:

- All medications both prescription and over-the-counter (even cough drops) require a specific school form filled out and signed by a licensed physician and the parent. Students are not allowed to carry medications on them without an order and the type of medications allowed to be carried with an order are limited. Most medications must be kept in the clinic. Please note we are not permitted to accept ANY medications without a school medical form signed by the physician and a parent. A new form is required each school year. 

- All prescription medications must be in their original container, with a pharmacy label containing the patient information on it and not expired. 

- All over-the-counter medications must be unopened, not expired, in their original container.

- Without the proper forms and the medication in the proper containers, we are unable to accept them to the clinic.

- All medications checked into the clinic will be signed and accounted for by both clinic staff and parent.
